You are write that iterative is several small waterfall projects, and that’s the good thing. Rather then getting down the entire path, there is opportunity to revise, ebb and flow.

The length of time is dependant completely on the project and the project requirements. The book Head First Software Development says 20 working days is a good guideline, but you should always customize that to the project.

I think the lines can blur and that Agile is really an approach (speaking from management not developer side). If you think about it an iterative approach, agile is just several small waterfall projects broken out. Some people take Agile to heart and are turning out nightly/daily builds that get rolled live on close to the same schedule. Maybe I’m off base but theoretically the lines can blur on a few different levels.
